Does what it's supposed to, and does it WELL! It will modify the 1st image's CODE and set it as the featured image, but most importantly WITHOUT RESIZING it in any way! Unlike the WordPress built-in featured image mechanism, when using it, it will add a RESIZED version of the image you're trying to set, making it all ugly and deformed. This plugin leaves the image as it is, and simply marks it as featured on the CODE LEVEL. This is the way it should be! Very well executed! Bravo to the developer! Honestly Wordpress should incorporate this plugin in their next release. This plugin is everything the built in "featured image" feature in Wordpress ISN'T but SHOULD BE!
